[Note: Click on highlighted forms for printable copies] Directions for College Applications: Teacher Letters of Recommendation - 1. Ask two or three teachers to write a letter of recommendation for you. 2. Give each teacher who has agreed to write a letter a Senior Information Sheet. Make sure you indicate the date by which the letter is needed. 3. Give each teacher who has agreed to write a letter a stamped envelope, addressed to the college admissions office so that they can mail the letter directly to the college (return address should be Holy Cross High School, 5035 Route 130 South, Delran, NJ 08075). Applications - 1. You should mail completed applications directly to the college. 2. Submit any Counselor forms or Secondary School Reports from the college to your counselor with any transcript requests (See below). 3. Make copies of your completed applications before mailing them. Keep a file of all your paperwork. Transcripts - All of the following must be submitted to your guidance counselor at least ten school days prior to any application deadline at the college. TRANSCRIPT REQUESTS WILL BE PROCESSED IN THE ORDER IN WHICH THEY ARE RECEIVED. 1. Submit the following forms to your counselor once. They will be kept on file: > Transcript Release Form (signed by parents) > Senior Information Sheet (available in Guidance Office) 2. Submit the following each time a transcript is to be sent: > Completed Parent Notification Card (from Guidance Office) > Four (4) first class postage stamps (loose) > One large manila envelope addressed to the college (9"x12") > $4.00 fee > Any forms from the college or scholarship program for the counselor to complete. Holy Cross will include your transcript, a counselor recommendation, our School Profile, and a return postcard and mail. NOTE: Faxxed transcripts are not official. There is an additional $1.00 fee for fax service. |
